We appreciate your referrals and enjoy working closely with you to ensure the best possible care for your patients.
- To refer a patient: Download our referral forms and fax them with your client and patient’s information. When we receive the paperwork, the client can schedule with us directly, or we are happy to contact the client ourselves. Please include medical records and lab work with the completed referral form. You may also refer a patient by phone during regular business hours.
- For urgent cases: If a case is urgent and evaluation is needed the same day, please call the closest DSVC office. If it is after regular business hours, please call the emergency surgeon pager number, provided in a separate mailing or please call any DFW area emergency veterinary hospital.
- To discuss a case with a surgeon: Call the hospital closest to you during regular business hours. We are happy to provide information prior to a referral.
- For review of patient radiographs: Use the radiograph referral form to provide a brief background of the case.
